Certificate of Progress
Please review the following information regarding Red River College Polytechnic’s immunization and testing record program requirements.
All healthcare students are required to have their immunization records reviewed by a healthcare provider to determine any outstanding requirements. Failure to meet these requirements in a timely manner may render you ineligible to complete your program.
Please print off the Certificate of Progress document and take it to a healthcare provider for a review of your current immunization and testing status.
Note: You are responsible for all costs associated with receiving immunizations and/or testing from your own healthcare provider.
Parents, partners, and close family members must not provide your immunizations and testing or complete any forms.
Once your review of outstanding immunizations is completed, please have your healthcare provider complete the certificate on the final page of the document.
Important: You must provide the completed certificate to your program to attend WIL.
